dental necessity, and as appropriate, government approval.

contract language, the contract language will control.
The purpose of dental policy is to provide a guide to coverage. Dental
policy is not intended to dictate to providers how to practice dentistry.
Providers are expected to exercise their clinical judgment in providing
the most appropriate care.

Each additional cast post – same
tooth (D6976):
To be used with D6970 or D6971
Each additional prefabricated post-same
tooth (D6977):
To be used with D6972
Policy/Criteria
Procedures are in accordance with generally
accepted standards of dental practice.
D6976 is considered included in D6970 or D6971.
Administrative Guidelines
Process to contract benefits.
- Procedure is considered inclusive. Therefore, an
additional post(s) is not covered.
- Procedure D6976 is considered inclusive. Therefore,
an additional post(s) is not covered.
- Procedure D6977 – Allow up to one more post.
