IMPORTANT INFORMATION
This Dental Policy has been developed through consideration of generally accepted standards of dental practice, review of dental literature, dental necessity, and as appropriate, government approval.

Benefit determinations should be based in all cases on the applicable contract language. To the extent there are any conflicts between these guidelines and the contract language, the contract language will control.

The purpose of dental policy is to provide a guide to coverage. Dental policy is not intended to dictate to providers how to practice dentistry. Providers are expected to exercise their clinical judgment in providing the most appropriate care.


Description

Extractions (D7111-D7250):
Include local anesthesia, suturing, if needed, and routine post-operative care. An extraction is a removal of one tooth using forceps and/or elevators, may require elevation of mucoperiosteal flap, removal of bone and/or sectioning for removal.

Coronal remnants - deciduous tooth (D7111):
Includes soft tissue-retained coronal remnants.

Extraction, erupted tooth or exposed root (elevation and/or forceps removal) (D7140):
Includes routine removal of tooth structure and closure, as necessary.

Surgical removal of erupted tooth requiring elevation of mucoperiosteal flap and removal of bone and/or section of tooth (D7210):
Includes cutting of gingiva and bone, removal of tooth structure, and closure.

Removal of impacted tooth – soft tissue (D7220):
Occlusal surface of tooth covered by soft tissue; requires mucoperiosteal flap elevation.

Removal of impacted tooth – partially bony (D7230):
Part of crown covered by bone; requires mucoperiosteal flap elevation and bone removal.

Removal of impacted tooth – completely bony (D7240):
Most or all of crown covered by bone; requires mucoperiosteal flap elevation and bone removal.

Removal of impacted tooth – completely bony, with unusual surgical complications (D7241):
Most or all of crown covered by bone; unusually difficult or complicated due to factors such as nerve dissection required, separate closure of maxillary sinus required or aberrant tooth position.

Surgical removal of residual tooth roots (cutting procedure) (D7250):
Includes cutting of soft tissue and bone, removal of tooth structure, and closure.

Policy/Criteria
Procedure are in accordance with generally accepted standards of dental practice.

Administrative Guidelines
Pay to contract benefits.

Surgical (D7210 & D7220) and bony extractions (D7230, D7240, D7241) are subject to review, requiring pre-treatment x-ray(s).
